Brief Nomination Summary – Required - submitted by the Nominator. Outlines why Nominee deserves to be considered for the Outstanding Citizen of the Year Award.
Nominee Biography – Required – submitted by the Nominator. Detailed biography that describes how Nominee is active in the community, outside of what the individual does for a living, if gainfully employed, and especially during 2020.
Supporting Information – Required – submitted by the Nominator. Examples of supporting information include, but not limited to:
Service club involvement (Lions, Rotary, etc.) and/or other community service activities;
Philanthropic activities;-Non-profit involvement;
Accomplishments that resulted in betterment of individuals and/or the community;
Other activities in the community (youth sports, etc.);
Letters of Recommendation - additional letters in support of Nominee are suggested from individuals other than Nominator.
The nominator of the individual selected to receive the award receives a complimentary ticket to the event!
